This course provide a general overview of forms used by Direct Services Providers providing supervision and support to children and youth placed in Special Care Settings for the Louisiana Department of Children and Family Services.  Participate will receive a brief description of each for include the purpose of the form and details regarding information needed to complete the form.  This course is recommended for all Direct Service Providers, Direct Service Provider employees, and caregivers that enter into an agreement with DCFS to provide services in Special Care Settings/Alternative Care placements.
